At the time of discharge from the hospital, many patients require additional rehabilitation therapies to prepare them to return to their own homes.

  • Physical Therapy
  • Occupational/Skills Therapy
  • Speech/Language Therapy

Rehab therapy is located in a recently renovated area of the Garfield unit at Rose Lane, which is easily accessible from the Main Lobby, as well as other rehab gyms within the facility. A transition room that simulates the surroundings of home is available to help short-term rehabilitation patients return to the home environment. The room includes a standard height bed, recliner, closet and home-like bathroom, including a bathtub.

Prior to admission, the individual's rehabilitation needs are assessed for appropriateness for the focused, short-term services offered. Services are available to:

  • Medicare
  • Medicaid
  • Private Insurance
  • Private pay residents, according to coverage guidelines
  • The average length of stay in Rehab Short Stay is less than 30 days, although this varies according to each resident's needs and progress. The Interdisciplinary Team, which consists of Physicians, Nurses, Therapists, Social Workers, Dietician, and Recreational Staff, meets routinely to evaluate progress. Additionally, the resident and designated family members actively participate in individualized care planning and communications.

     

    When residents no longer need Rehab Short Stay services, they are appropriately transitioned to their homes.
